What do I need to do to get the reward?
Complete the referral form below and receive £750 or a Tassimo coffee machine when your referred organisation joins us. Referrals are unlimited so you can refer as many companies as you like and you will be rewarded for every surgery that switches to Premier Choice.

What happens next?
Once you have submitted the information to us, one of our Sales Executives will set up an appointment with them. We will keep you updated with the progress of your referral and let you know when your rewards are on the way!

Terms and Conditions

  1. The following Terms and Conditions apply to the PCT Referral Scheme (the ‘Scheme’). You shall be deemed to have accepted these Terms and Conditions in full when you submit a referral to us in accordance with clause 4.
  2. The Scheme is open to existing customers only. New customers may qualify for a reward if they switch to us – please refer to www.premierchoicetelecom.com/switch for more details.
  3. Subject to the other provisions in these Terms and Conditions, if you refer a new customer to us, you will be eligible for a reward if the organisations you refer switches to Premier Choice within three months of the date of your referral. You will receive one reward for each referral and the reward you receive will depend upon the contract the surgery signs up for.
  4. To be eligible for the Scheme you must submit details via our online form which is available at www.premierchoicetelecom.com/referral. Entries made by any other method may not be accepted. There is no limit to the number of referrals you can submit and you will be rewarded for each successful referral.
  5. Entrants must be aged 18 and over.
  6. The reward will be provided to the organisation which is an existing customer, not the individual making the submission. In entering the Scheme, you agree that you do so on behalf of the organisation you are employed by or are otherwise affiliated with.
  7. The organisation you refer must be located within the UK.
  8. If your referral is successful, you will be contacted by your Account Manager and you will receive your reward within 60 days of the surgery you refer meeting the qualifying criteria.
  9. You will be eligible to receive a coffee machine and a limited supply of pods if the organisation you refer signs up for a two-year minimum contract for lines and calls only (minimum £50 plus VAT pcm).
  10. The coffee machine reward under clause 9 will be a Tassimo Vivy T12 model retailing at £99.00 from www.tassimo.co.uk (price correct as at 21 June 2017). We retain the right to substitute the reward with another of a similar kind and value in the event that the original reward is not available.
  11. You will be eligible to receive a £750 e-supercheque if the organisation you refer signs up to a three-year minimum contract for lines and calls including phone system equipment.
  12. The £750 reward under clause 11 will be issued as a Grass Roots E-Supercheque which can be redeemed for a wide choice of vouchers and gift cards from all major high street retailers, experiences and travel and entertainment suppliers.   13. The coffee machine reward will be delivered to a UK address by us during a site visit, or posted where no site visit is required. E-supercheques will be delivered to the email address provided on the online form or to any other email address we hold for you.
  14. Submissions will be processed in the order they are received and in the event that the same organisation is referred by more than one customer, the reward will be provided to the customer who submits the referral first. If the organisation you refer submits its details in accordance with our Switch Scheme and you also refer its details to us under this Scheme then only the first entry received will be eligible for a reward.
  15. Rewards are non-exchangeable, non-transferable, and are not redeemable for cash.
  16. In the unlikely event that the coffee machine you receive is faulty, you must notify your Account Manager within 30 days of the date on which you receive the machine. We will send a replacement machine, subject to stock availability, within 30 days of the faulty machine being received by us. Whilst we are able to exchange a faulty coffee machine which meets the criteria set out above, we are unable to replace a coffee machine where faults arise from normal wear and tear, accidental damage or for any other reason.
  17. In the unlikely event that your e-supercheque does not work, you must notify your Account Manager within 30 days of the date on which you receive the e-supercheque. We are able to replace the e-supercheque if the code provided is faulty but not for any other reason.
  18. If we have any reason to believe that there has been a breach of these Terms and Conditions, we may, at our sole discretion, reserve the right to exclude you from participating in the Scheme.
  19. We reserve the right to hold void, suspend, cancel, or amend the Scheme at any time.
  20. Insofar as it is permitted by law, we will not in any circumstances be responsible or liable to compensate you or accept any liability for loss, damage, personal injury or death occurring as a result of the Scheme except where it is caused by our negligence.
  21. After we have sent the e-supercheque to you, we will not in any circumstances be responsible or liable for any loss, fraudulent use or if, for any reason, the e-supercheque cannot be used.
  22. You must obtain the consent of the organisation you refer before making the referral and you agree that we may use the name of your organisation along with the name of any individual making the referral in our communications with the organisation you refer.
  23. We will hold and use any personal information supplied by you in accordance with our privacy policy, a copy of which is available at www.premierchoicetelecom.com/privacy-and-cookies.
